Consent to Photograph and Interview

I authorize Big Bend Hospice, Inc. (BBH, Inc.) to use my photograph/interview/story/testimonial with my full knowledge and consent for the purpose of News, Public Relations, Publicity, or Fundraising. I have entered into this agreement in order to assist education, public relations, and charitable goals and hereby waive any right to compensation for such uses. The term “photograph” shall mean motion picture or still photography in any format, as well as videotape, video disc, and any other mechanical means of recording and reproducing images.

BBH, Inc. and any photographer/story collector assigned by the agency’s staff are hereby relieved and released of all responsibility or claims in regard to the above.
