Admissions Specialist, RN – FT

Supervisor: K. Fenior

The Hospice RN (Registered Nurse) for Admissions performs pre-admission evaluations to determine appropriateness for hospice services. Initiates the admission process by educating and informing patients and families of hospice and its services. Must have an active Florida RN License, a Bachelor of Science in Nursing degree is preferred. Minimum of 2 years of nursing experience required. This position will work full-time hours, which may include weekends and/or holidays. 

Community Engagement Coordinator – FT

Supervisor: K. Mandell

This position recruits, trains, and supervises volunteers who support the mission of Big Bend Hospice. This position is responsible for monitoring volunteer assignments, evaluating performances, providing ongoing training, and assisting in creating volunteer recognition opportunities. This position serves as a liaison between volunteers, staff, and the community at large, assisting in community relations in Taylor, Wakulla, and Franklin Counties. They also serve as a patient-family advocate to find how volunteers can help increase the patient’s quality of life. A Bachelor’s Degree is required, or consummate experience in volunteer coordination is preferred. A valid Florida driver’s license, reliable transportation, and current automobile insurance are required.


Supervisor: T. Dooley

This position will work weekdays and weekends on a rotating schedule of 6-8 hours each day, for a total of 40 hrs/week. The primary role of this position will be to prepare tasteful meals that are consistent with the patient’s plan of care and nutritional needs for up to 12 patients per day. Also responsible for catering services for staff meetings and events. Housekeeping duties will include cleaning common areas, cleaning patient rooms and preparing them for occupancy, and other housekeeping duties as assigned. A High School Diploma with a minimum of 1 year of experience cooking in a commercial and/or healthcare facility. This position will be required to successfully complete the Food Services Manager Certification within the first 30 days.

Licensed Practical Nurse (LPN) – FT

Supervisor: TBD

Under the supervision of an RN, the LPN provides nursing care to patients and families in their home, incorporating psychosocial, cultural, physical, spiritual, and biological components and appropriate nursing intervention as identified in the patient/family plan of care, developed by the IDT. Demonstrates clinical skills regarding the application of Hospice Clinical and Administrative policy and procedures such as pain medication titration, symptom management, and patient/family relationships. This position communicates the progress and needs of patients and families to the other interdisciplinary team (IDT) members, including the physician. Current Florida licensure as a Licensed Practical Nurse. Minimum of three years of experience in medical/surgical nursing. Hospice/Home Health experience preferred. The schedule for this position is Monday through Friday – no weekends.

Music Therapist – Float

Supervisor: S. Droessler

This Board-Certified Music Therapist will provide care to patients throughout Big Bend Hospice’s 8 county service area and in new lines of service as needed. This full-time salaried and benefitted position will “float” across BBH’s entire service area providing coverage when other FT staff are on leave or PTO and coverage when patient care needs and census increases. The selected candidate will be highly flexible, have strong communication and assessment skills, and should be prepared to take initiative in unfamiliar situations. Responsibilities include planning, implementing, and documenting music therapy interventions with hospice patients from a variety of age groups and their family members, working as part of an interdisciplinary team, and collaborating with other music therapy staff. Occasional evening or weekend hours for community events and bereavement groups as assigned. A valid driver’s license, reliable transportation, and current automobile insurance are required. MT-BC (music therapist board certified) certification through the Certification Board for Music Therapists (CBMT) or current eligibility for the exam with subsequent successful completion required. Strong musical skills and clinical understanding required; hospice experience and experience with electronic medical record system documentation preferred. After a requisite period of agency and clinical experience, the selected candidate may also be assigned to student supervision and other duties as assigned. A minimum of 3 years of CNA experience.

Nurse Practitioner (APRN) – FT

Supervisor: N. Suchak

This is a full-time nurse practitioner (NP) position. The NP will work in collaboration with the Hospice Physician and Hospice interdisciplinary team (IDT) to provide hospice services to patients and families in an inpatient, home, or facility setting. Responsibilities include: maintaining a regular schedule of clinical participation for the BBH Service area and sharing call schedules with other medical providers to provide 24-hour, 7-day/week coverage for BBH patients; performing face-to-face visits as part of the (re)certification process and helping develop and implement plans of care; assist in developing management plans for pain and other symptoms; perform required documentation and provide education to patient/families. Performs other duties as requested by the Medical Director and/or CEO. Stays abreast of all state and federal regulations/statutes and licensure requirements. Must have a current APRN License and a Master’s Degree from an accredited Nurse Practitioner Program. BLS is also required and 1 year of APRN experience is preferred. Previous Hospice experience is also preferred.

Referral Specialist – FT

Supervisor: T. Christian-Griffin

This is a full-time position that will work rotating weekends and holidays. Experience with medical supplies, records, and terminology is preferred. Responsible for referral calls concerning hospice care and process requests for information and visits by Admission Specialists. Must be highly organized with excellent communication skills with 3-5 years administrative experience in a healthcare environment including insurance verification and/or medical billing.

RN BBH Leon Team – FT

This position will provide care for patients throughout Leon County. Must have a current Florida RN license and a minimum of 3 years RN/LPN experience preferably in hospice or home health.

RN BBH Coastal/Franklin Team – FT

This position will provide care for patients throughout Franklin County. Must have a current Florida RN license and a minimum of 3 years RN/LPN experience preferably in hospice or home health.

RN- Care and Comfort Team – FT

Supervisor: T. Agboola

This position will provide care for patients in ALS, nursing homes, and other facilities throughout Leon County. Must have current Florida RN license. BSN preferred. Minimum of three years of nursing experience, preferably in hospice or home health.

RN – Hospice House – FT Nights

Supervisor: T. Dooley

This position will work primarily nights, (3) 12 hour shifts for the Hospice House including weekends and holidays. Must have current a Florida RN license and a minimum of 3 years RN/LPN experience, preferably in-home health or hospice.

RN/LPN On-Call Team (2) – FT

Supervisor: M. Ramos

This is a full-time RN position for the On-Call team. Qualified applicants must have a current Florida RN license. Required experience includes a minimum of 1-year in-patient nursing experience with previous hospice or home health experience preferred. This position will provide On-Call coverage for the BBH 8-county service area for 7 consecutive days followed by7 days off. The schedule for this position is Thursday through Wednesday, with the hours of 5 pm to 8 am weekdays and 24-hour coverage on weekends. This position includes the use of a company vehicle as well as a shift differential.

RN/LPN Quality Assurance

Supervisor: F. Bell

This is a full-time position that will assist with HIS, Infection Control, ADR’s, Utilization Review, and other related duties as part of the BBH Compliance Team. Will also provide chart review for
nursing and other disciplines. This position will also serve on Eligibility and other committees as assigned. Must have Hospice experience and a current RN/LPN license preferred. Other healthcare-related licenses or a Bachelor’s degree in a healthcare-related field will be considered.

RN BBH Coastal/Wakulla Team – FT

This position will provide care for patients throughout Wakulla County. Must have a current Florida RN license and a minimum of 3 years RN/LPN experience preferably in hospice or home health.

Senior Accountant – FT

Supervisor: N. Bell

The Senior Accountant role at Big Bend Hospice will act as a key point for our accounting data reconciliation and reporting. This is a full-time position within the Finance Department of BBH. Key duties of this role include data retrieval, grant reporting, grant reconciliations, approving invoice batches, posting donations, 403 B reporting, bank statement reconciliation, and managing various expense allocations. This position requires a bachelor’s degree in accounting and 3-5 years of public accounting experience. Preference will be given to applicants with previous experience in hospice or other non-profit/health care environments. Applicants must be well versed in GAAP (Generally Accepted Accounting Principles) and experience in Sage Intacct or similar General Ledger Software is a plus.

Social Worker – FT – Coastal Team

Supervisor: K. Solberger

Full-time social work position for the BBH Coastal team, which includes Leon 05 zip code. Must have a Master’s degree in Social Work, LCSW preferred. Two years of home health or hospice experience preferred. Responsibilities include active and cooperative engagement with members of the interdisciplinary team to facilitate optimal patient and family coordination of care. Provision of psychosocial assessments to support the expressed wishes of patients/families and identify unanticipated needs to help patients/families make informed decisions. Empower patients and families through the provision of education related to end-of-life care, advanced directives, long-term care planning, safety, and financial benefits for which a patient or caregiver may be entitled.

Spiritual Care Counselor – FT

Supervisor: K. Snow

This position is responsible for providing spiritual counseling and support to hospice patients and families in accordance with their plan of care. Duties include documentation on the plan of care, counseling and spiritual support to hospice patients and families, attending team meetings and participating in treatment planning, and networking with community clergy to share information about hospice services as assigned. Will be required to complete ongoing education requirements and perform other duties as assigned. This position requires excellent interpersonal and communication skills, the ability to work well with diverse groups and diverse healthcare systems and to adapt to a variety of changing circumstances and settings. Denominational or faith community endorsement is strongly preferred. Must have Master of Divinity degree or equivalent (i.e. M.A.C.E., M.A.T.S., Th. M) from an accredited seminary or theological school and Clinical Pastoral Education within a health care setting (1 unit minimum required, 4 units preferred).

Licensed Practical Nurse (LPN) – FT – 7 Oaks

Supervisor: K. Broussard

The LPN will provide telephonic care coordination with the patient, caregiver, and the interdisciplinary team; this includes triaging incoming calls and making proactive calls as requested by the Nurse Practitioner. This position provides real-time communication between the patient and provider to allow for treatment in a timely manner. Responsibilities include receiving referral information and documenting in EMR information received and collaborating with other providers to obtain necessary health history information for patients accepted to Transitions Supportive Care (TSC); conducting appropriate triage and escalation of routine, urgent and emergent patient calls; clinical care coordination with patient-family and other providers and specialists including NP home visits, PCP or specialist office visits appointment coordination, initiating clinical service orders on behalf of the patient and family; provide accurate and timely documentation of supportive services to reflect the patient’s plan of care; and participate in the coordination of supportive services, appropriately reporting the identified needs to the interdisciplinary team.
Education: Graduate of an accredited practical nurse or vocational nursing program.
Experience: Three years of nursing experience. Community health/hospice or palliative care experience preferred. Currently licensed as an LPN in the State of Florida.

Medical Social Worker (LCSW) – FT – 7 Oaks

Supervisor: S. Davidson

The TSC Medical Social Worker assesses the psychosocial status of patients and families/caregivers related to the patient’s life-limiting illness and environment and communicates findings to the TSC NP and other members of the TSC interdisciplinary group.  They will provide an assessment of the patient’s identified residence and assistance when this is not safe; carries out social evaluations and plans intervention based on evaluation findings; provide counsel to patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Assists TSC team members in understanding significant social and emotional factors related to chronic health problems and death/dying issues and maintains collaborative relationships with organization personnel to support patient care. 
Licensure/Certification/Registration: Current or pursuing to obtain w/in three-month period upon being hired, valid license as a clinical social worker (LCSW) issued by the Florida State Board of Social Work Examiners  
Education: Master’s Social Work from accredited program 
Experience: Minimum 1-year healthcare experience; Hospice/Palliative experience preferred. Documented clinical experience appropriate to the counseling and casework needs of the seriously ill. Demonstrates good verbal and written communication, and organization skills. Possesses and maintains current CPR Certification as required by organizational policy. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. 

Nurse Practitioner (APRN) – FT – 7 Oaks

The Transitions Supportive Care Nurse Practitioner provides palliative care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications, or treatments, coordinating, and collaborating with patients, families, and other healthcare practitioners. In compliance with the state Nurse Practice Act Under the supervision of a physician, the nurse practitioner visits patients (in-home, facility, or clinic setting), completes physical assessments, performs face-to-face assessments, and evaluates palliative/hospice eligibility as assigned in consultation with an interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities, overseeing the patient’s care in conjunction with the interdisciplinary team and other healthcare practitioners.
Required Education and Experience:
• Licensed in Florida as an Advanced Practice Registered Nurse; certified as a Nurse Practitioner. Must have BLS certification.
• Master’s degree with a minimum of two (2) years of Nurse Practitioner experience and a minimum of one (1) year palliative or hospice care experience preferred.
• Knowledge of principles of palliative/hospice care and the end-of-life process; Certification in a specialist area preferred; (e.g., Hospice and Palliative Nursing (ACHPN), Pediatrics or Geriatrics).
