Skilled Home Health Care for Patients in North Florida

Big Bend Home Health provides short-term skilled medical care for patients recovering from surgery, illness, injury, hospitalization, or a change in condition.

Our goal is to help patients continue healing safely at home while supporting continuity of care after discharge or referral.

We work closely with referring providers to develop individualized home health care plans that support the patient’s clinical needs and meet eligibility requirements when possible.

Appropriate Home Health Referrals

Patients may benefit from home health care when they require skilled support but do not need to remain in the hospital. Services may be appropriate for patients who need nursing care, rehabilitation therapy, education, wound care, medication support, or monitoring after a recent health event.

To qualify for insurance-covered home health services, patients typically need:

☑ A skilled nursing or therapy need
☑ Homebound status
☑ A safe home environment

Coordinated Care at Home

Big Bend Home Health provides skilled care in the patient’s home based on their individualized care plan and physician orders.

Depending on the patient’s needs, visits may include nursing care, therapy, education, or other skilled services one or more times per week. Our team helps patients continue recovering in a familiar setting while supporting safety, comfort, and progress between provider visits.
