About Palliative Care

Palliative care provides added support for people living with a serious illness. It is meant to help patients and families better understand their care, manage symptoms, and feel more supported as medical needs change over time. Palliative care can be provided alongside ongoing treatment and focuses on the whole person, not just the diagnosis.

At Big Bend Palliative Care, we are here to help you navigate your healthcare journey with compassionate guidance that is centered on your needs. Our team works alongside patients, families, and medical providers to improve communication, connect people with helpful resources, and offer support throughout the course of illness.

We want each patient and family to feel supported and cared for every step of the way.

A Team Approach to Care

Our interdisciplinary team includes our medical director, nurse practitioners, LPNs, licensed clinical social workers, community navigators, trained volunteers, spiritual care counselors, and veteran liaisons. Each team member plays an important role in addressing the clinical, emotional, practical, and spiritual needs that can come with serious illness.

By working together, we are able to provide an added layer of support that helps patients and their loved ones feel more informed, connected, and cared for.

Clinical Focus

Our Nurse Practitioners provide clinical support and serve as a liaison between the patient and their medical providers. They help review medications, explain diagnoses and treatment plans, and educate patients and families about their specific condition so they can feel more informed and supported. By improving communication across the care team, Nurse Practitioners help ensure the patient’s goals, concerns, and needs remain at the center of care. Working together, we are able to provide an added layer of support that helps patients and their loved ones feel more informed and cared for.

Community Navigation

When facing a serious illness, it can be difficult to know where to turn for help. Our skilled Community Navigators assess each patient’s needs and connect them with appropriate local resources and support services.

They can also assist with completing forms for certain programs or services, helping patients and families access support more easily. Their role is to ease practical burdens and guide patients and loved ones toward resources that can improve quality of life.
